Profile:
This 79-year old African-American female, presents in your office for an examination. You have not seen this patient in the past. She has visible physical limitations and walks with a cane. She is 5’ 7” and weighs 200 lbs. She is accompanied by her daughter as well as her 12-year old granddaughter.
She indicates she has not been to see a dentist for 3 years.
Chief Complaint:
“My teeth have not been cleaned in a long time and are in bad shape.”
Medical
History:
The patient had a hysterectomy at age 62. She suffered a mild stroke four years ago, relies solely on a cane in order to walk and occasionally suffers with shortness of breath upon exertion. She recently underwent laser surgery on the retina in her right eye.
She indicates she gives herself insulin injections daily and reports that her HbA1c values are typically between 8.0-9.0%.
She reports numerous co-morbidities including diabetes, GERD, hypertension, hyperglycemia, and osteoporosis.
Daily
medications:
81 mg aspirin, daily insulin injections, (Omeprazole) Prilosec®, (Lovastatin) Mevacor®, (Atenolol) Tenormin®, (Ramipril) Altace®, (Clopidogrel bisulfate) Plavix® and (Risedronate sodium) Actonel®. HbA1C is 8.0-9.0%.

Blood Pressure: 120/80; Pulse: 85 bpm; Respiration: 20 breaths/minute
Dental History:
The patient reports she had the mandibular right first molar extracted 4 years ago due to an abscess and has received no dental treatment
